Tandem Survey - Questionnaire

This form can be filled in by everyone who owns a tandem. Owners of more than one tandem please fill in one questionnaire for each bike. Tandems which are no longer in use may also be described. Triplets, recumbents, semi-recumbents etc. are welcome, too

The questionnaire is divided into three parts: Part 1 covers the basics. You will be asked for the bike model and what it is used for. Part 2 asks for some more tandem-specific data but does not go into too much depth. Part 3 contains a long list of technical details. Please note that it is not mandatory to answer all questions of part 2 and part 3. For all technical questions please describe the current condition of the bike or the last status of a bike you do not ride any more.

User's Manual Radio buttons and selectors allow only one answer. Check boxes allow one ore more than one answers. Text fields can be filled in. If there are given two fields using different units of measurement (e.g. centimeters or inches) decide for one of them. Questions which do not match: please just skip. After filling in the form please submit by pressing the "Submit"-button. The whole questionnaire can be cleared by pressing the refresh button of your browser.
